
NDIS Services
I offer services to adolescent and adult NDIS participants who are self or plan managed on the Sunshine Coast and surrounding areas. I'm experienced in working collaboratively and compassionately with NDIS participants, their families and their supports to help NDIS participants achieve their goals and get the most out of their NDIS plans.
My NDIS services include but are not limited to:

Functional Capacity Assessment
Functional Capacity Assessments are completed for NDIS participants to ensure they have the right level of funding to achieve their goals, support their needs and live a more fulfilling life.
My approach is to ensure that the purpose of the assessment is clear and is undertaken in an environment where you feel safe and supported within an agreed upon timeframe. The assessment will be completed using objective and standardised measures when required, taking into account the NDIS participants function, current supports and goals.
Assessments include a face to face component, liaison with other parties (when consent is provided) and a report that follows. The report will provide advice on the type of equipment, home modifications, therapy and carer support the NDIS participant requires to meet their goals. The report will assist with the process of decisions regarding the type and level of funding for supports under an NDIS plan.


Therapeutic Interventions
I believe that everyone has the right to participate in tasks that are meaningful to them. I have experience in using a wide range of evidence based approaches to help NDIS participants overcome barriers and achieve their goals to live a more fulfilling life.

Telehealth
Telehealth is an effective method for providing occupational therapy services without requiring travel or face to face appointments. Telehealth services use digital technology such as a smart phone or tablet with a video call to connect NDIS participants with health professionals, remotely. Telehealth can have similar outcomes when compared with in-person allied health support for eligible participants.
To participate in telehealth services you will require internet connection, a suitable device with a camera and microphone e.g. smartphone, tablet, laptop or desktop computer.
To determine if this is the most appropriate service for you, an initial phone conversation or initial assessment would be conducted to establish suitability.



Home Modifications
Home modifications are changes to the structure, layout or fittings in your home to make it more accessible. Whether it be minor or major modifications, it can significantly improve a persons independence and quality of life.
I am skilled at assessing a person and their home environment to make recommendations to help NDIS participants achieve their goals. I have completed the necessary training to provide complex home modification recommendations for NDIS participants (CPPACC402A - Provide access advice on building renovations, CPPACC5016A - Provide expert access advice on renovations in private dwellings).
I work in consultation with local builders to ensure that all modifications are in line with the Australian building standards - design for access and mobility AS1428.1.

Assistive Technology
There is a wide range of assistive technology and daily living aids that can assist NDIS participants to maximise their independence at home and in the community. I work closely with local providers who can offer equipment trials in store, or in home to assist NDIS participants to achieve their goals.

Specialist Disability Accommodation (SDA)
SDA is a range of housing designed for people with extreme functional impairment or very high support needs. SDA dwellings have accessible features and require a high level of property modification to help residents live more independently and allow other supports to be delivered better or more safely. This might include wheelchair accessible bathrooms, ceiling hoists, home automation e.g. voice activated lights and accessible bench tops for cooking.
To determine suitability for SDA, a thorough assessment is completed to cover all aspects of an NDIS participants functional capacity, carer support needs, equipment and environmental requirements to enable the NDIS participant to live as independently and as safely as possible.
A report is completed to document all aspects of the NDIS participants specific and unique requirements with use of the SDA design standard to demonstrate the need for SDA.
Supported Independent Living (SIL)
SIL is one type of support to help NDIS participants to live in their home. It includes help or supervision with daily tasks to help NDIS participants live as independently as possible, while building their skills. SIL is for people with higher support needs who need some level of help at home most of the time.
To determine suitability for SIL, an assessment is completed to document the level of assisted living an NDIS participant requires. This includes information about how a person transfers, their mobility, ability to complete self care and domestic tasks, challenging behaviours, ability to look after finances and medications and how they access medical appointments. This is to determine the level of support an NDIS participant requires to live safely and independently.
A report is completed and submitted to the NDIS to document all aspects of the NDIS participants goals and care needs to demonstrate the need for SIL funding.
